Who we are

This website is operated by Hoan Health, Inc. It describes the HOT clinical program and software for licensed practices. The website is informational. It is not a clinical service and it does not provide medical advice.

What this website collects

The only form on this website is a provider inquiry form. It asks for your name, role, practice name, city and state, work email, an optional phone number, an optional referral source and an optional message of up to 500 characters. It does not ask for patient information, health information, dates of birth, diagnoses, medications or clinical records. Please do not send health information through this website.

How an inquiry is delivered

Form submissions are delivered by email to the HOT team through a form delivery service. The website keeps no database of its own. The delivery service may store a submission for a limited time to deliver it and may keep a delivery log for security and troubleshooting.

Cookies and analytics

This website does not use cookies for advertising and does not run analytics. If analytics are added later, this policy will be updated before they are turned on.

How we use information

We use provider inquiry information to respond to your request, to discuss HOT for your practice and to keep records of those communications. We do not sell your information and we do not share it for advertising.

Service providers

We use service providers for website hosting, form delivery and email. They receive only the information needed to provide those services. The Data Processing Agreement for the HOT platform is a separate document for practices and does not apply to this informational website.

Retention and deletion

We keep provider inquiries only as long as needed to respond and to maintain business records. To ask us to correct or delete an inquiry, contact us at providers@hoanhealth.com. We may need to verify a request before acting on it.

Children

This website is intended for licensed practices and adults. It is not directed to children and we do not knowingly collect information from children.

Changes

We may update this policy as the website or our services change. The date at the top of this page shows the latest revision.
